Patient Information
Current Patients:
Please arrive 10 minutes before your appointment. If any personal or insurance information has changed since your last visit, please tell the receptionist when you check in. This lets us update your records and file your insurance with the right carrier.
New Patients:
Please arrive 30 minutes before your appointment. This gives you time to fill out your new patient forms before you see your doctor, and for us to get all your information into our system before your appointment. Or if you prefer, you can download the forms here on our website, print them, fill them out ahead of time, and give them to the receptionist when you check in. In either case, bring your insurance card with you when you come.
Rescheduling Appointments:
We understand that sometimes it’s necessary to reschedule an appointment due to circumstances beyond your control. If that should happen, we ask that you kindly contact us at least 24 hours prior to your scheduled time and we’ll be happy to reschedule your visit.
