This gives you time to fill out your new patient forms before you see your doctor, and for us to get all your information into our system before your appointment. Or if you prefer, you can download the forms here on our website, print them, fill them out ahead of time, and give them to the receptionist when you check in. In either case, bring your insurance card with you when you come.
We recommend scheduling an appointment to reduce wait times, but we also accept walk-ins for urgent medical needs. Please call ahead if possible so we can prepare for your visit.

Many health concerns can be handled quickly and safely at our clinic. If you are experiencing life-threatening symptoms such as chest pain, shortness of breath, or uncontrolled bleeding, call 911 immediately. Things we can handle at the clinic - same-day appointments and on-call nurse 24/7. After hours and holiday hours are also covered by an on-call nurse. Give us a call first and see if we can help.
